What you need to know for your first visit
FollowMyHealth® Patient Portal
FMH messages are reviewed M – F 8am – 5 pm (excluding holidays) and may take up to 72 hours to be returned.
For medical emergencies, call 911. For URGENT MEDICAL concerns, please call our office and select option 1.
Forms sent outside of a Well Visit will incur an administrative charge of $25 and may take up to 5-7 business days to be completed. When available and requested, forms can be expedited (24 to 48 hours Mon-Fri) and will incur a fee of $50 per form.
